Job description
TeamCare Medical in New York is seeking a Nurse Practitioner Supervisor for the Mobile Program to provide oversight and coordination of participant care with the Interdisciplinary Team. The NPS will liaise among participants, community physicians, nursing homes, and plan staff to develop and implement care plans.
The role requires NY NP and RN licenses, DEA, and board certification; minimum 2+ years home care or care management, strong communication, and ability to manage multiple projects.
