Job description
21st Century Home Health Services (21HHS) is seeking a Full Time Physical Therapist Case Manager for Santa Clara County, covering Gilroy/Morgan Hill. You will evaluate and treat patients in their homes, develop care plans, and coordinate with physicians to ensure high-quality, compassionate care.
