Job description
Optum Home & Community Care is seeking an NP or PA to perform in‑home assessments and coordinate preventive care for patients in their own homes. You will evaluate medical history, medications, and vital signs, and perform comprehensive physical exams in a home setting.
The role offers flexible scheduling (20–40 hours/week) with no on‑call, no weekend, and no holiday requirements, supporting work‑life balance while delivering compassionate, integrated care.
