Job description
CareSource in Massachusetts is seeking an Integrated Care Clinical Manager to oversee care coordination for dually-eligible enrollees with complex needs. This RN-led role works with the ICT to develop holistic plans, coordinate services, and monitor long-term care utilization across community settings.
