Job description
Brigade Health is seeking a compassionate Nurse Practitioner or Physician Assistant to join our Mobile Primary Care Team, delivering high-quality medical care to older adults where they live and thrive. You will conduct comprehensive assessments, develop individualized care plans, manage chronic conditions, and coordinate with a multidisciplinary team across home and senior-living settings.
