Job description
MNRAAA is seeking a full-time Person-Centered Counselor to assist Veterans in choosing caregivers, services and supports, helping them live safely at home within their budgets.
The PCC will assess functional status, develop and monitor a care plan, coordinate with VA facilities, and ensure ongoing service quality. A valid driver's license and reliable car are required. This mobile role requires travel within the region.
