Job description
Enhabit Home Health & Hospice seeks a qualified Social Worker to administer social services for patients receiving intermittent professional care. You will educate patients, families, and health care team members while following physician orders and the plan of care.
The role requires an MSW and state licensure, with at least one year of health care social work experience. Medicare home health or hospice experience is preferred, and strong technology skills are expected.
