Job description
Novant Health is seeking a Case Manager MSW to lead patient assessment and complex discharge planning at New Hanover Medical Center in North Carolina. You will collaborate with physicians and the care team to create proactive and safe discharge plans, coordinate care, and support psychosocially complex cases while ensuring alignment with Novant Health policies and values.
The role requires a Master’s in Social Work from a CSWE-accredited program, 2+ years of relevant experience, and licensure
