Job description
Enhabit Home Health & Hospice is seeking a licensed Master of Social Work (MSW) to provide intermittent professional social services to patients in the home setting. You will collaborate with patients, families, and the health care team to implement the plan of care as directed by physicians.
The ideal candidate has an MSW or LMSW, state licensure, and at least one year of relevant healthcare social work experience, with solid technology skills including mobile device use.
