Job description
Baptist Health is seeking a Discharge Planner (MSW) to coordinate care for a caseload with physicians, nursing staff and other health professionals, ensuring efficient use of resources and advocating for patient and family needs through assessment, planning and discharge transitions.
Applicants should have a Master’s in Social Work and at least 3 years hospital experience; LMSW/LICSW preferred or obtainable within 12 months. Strong MS Office and teamwork are essential in this inpatient setting.
