Job description
Elara Caring seeks a PRN Medical Social Worker to support home health patients and families across Portland and southern Maine. You will work independently in patients’ homes, collaborating with an interdisciplinary team to remove barriers and advocate for optimal home-based care.
You’ll provide counseling and connect families with community resources, while developing Plans of Care and ensuring documentation is timely and accurate.
