Job description
Enhabit Home Health & Hospice is seeking a qualified social worker to provide patient-centered social services in a home health setting. You will educate patients, families, and care teams, and perform services according to physician orders and the plan of care.
Applicants must hold an MSW from a CSWE-accredited program, be licensed in the state, and have at least one year of healthcare social work experience. CPR certification is required.
