Job description
Clearway Health is seeking an MTM Clinical Pharmacist to optimize outcomes for patients on chronic and specialty therapies. You will conduct medication therapy reviews, provide disease-specific counseling, and collaborate with prescribers, nurses, and payors to initiate and sustain therapy.
You will support prior authorizations, monitor outcomes, and document interventions to support quality and compliance.
