Job description
Qualifications:
One-year experience in hemodialysis required.
Graduation from an approved School of Nursing.
CA Registered Nurse License - Required.
American Heart Association BLS Provider - Required.
Ability to work under pressure and demonstrate leadership potential.
Must be able to complete training and demonstrate competencies in machine setup, initiation, and termination of treatment, monitoring of patients, and other functions necessary to complete a dialysis procedure.
Duties and Responsibilities:
Patient Treatment Activities:
1. Appropriately manage and direct care of fistula, shunts, grafts, and dialysis catheters. Understand care procedures and appropriate actions during access problems. Perform venipunctures, shunt care, and dressing changes with expertise.
Responsible for proper management of dialysis treatment for all patients under care.
Administer and document all medications. Assess patient needs for PRN medications and respond accordingly.
Document assessments, evaluate, and respond to adverse reactions.
Obtain and execute physician orders promptly and accurately.
Perform lab procedures, including requisitioning, centrifuging, and obtaining specimens. Handle blood specimens appropriately, using universal precautions, and work with hepatitis and other bloodborne pathogen carriers following isolation precautions outlined in the Procedure manual.
Implement individual patient care plans as prescribed by the physician and evaluate outcomes continuously.
Understand and interpret lab values. Explain issues to patients and staff when necessary.
Observe and evaluate patient signs, symptoms, and reactions to therapy; implement appropriate actions.
Instruct patients or relatives on home care, following physician instructions.
Maintain medication logs.
Supervise technicians to ensure completion of assignments.
Make frequent patient rounds to ensure safety.
Other Activities Related to Patient Care:
Ensure all equipment tests are completed before dialysis initiation.
Perform spot checks on machines and patients during dialysis to ensure safety and quality.
Check the crash cart daily, as per policy.
Additional Responsibilities:
Assist in maintaining inventory of stock medications.
Perform and document initial nursing assessments on newly admitted patients.
Demonstrate teaching abilities related to treatment procedures.
Understand, perform, and direct all emergency procedures.
Assist in training and evaluating new employees.
Maintain an empathetic attitude with patients, counsel, and encourage compliance.
Set an example of professionalism; support and promote Center policies.
Report defective equipment and maintain a safe environment.
Report inappropriate conduct or behavior and take corrective actions if necessary.
Follow established safety practices and universal precautions.
Assist in implementing rules and regulations for staff, patients, and visitors.
Maintain confidentiality of patient information and medical records.
