Job description
Hartford Healthcare is seeking a Neighborhood Health professional to support community-based care across our Connecticut network. In this role, you’ll coordinate outreach, deliver basic primary and preventive services, and connect patients to needed medical and social resources. You’ll educate individuals and families on wellness and chronic disease management while maintaining accurate electronic health records. Working in a collaborative, inclusive culture, you’ll partner with multidisciplinary teams, leverage cutting-edge tools, and access robust professional development as you improve health outcomes in local neighborhoods.
