nnJob Summary
nThe Nephrology Nurse Practitioner (NP) provides advanced, patient-centered care to individuals with acute and chronic kidney disease across inpatient, outpatient clinic, and dialysis settings. The NP collaborates with nephrologists and interdisciplinary teams to optimize outcomes, manage complex renal conditions, and promote kidney health.
nnKey Responsibilities
nClinical Care
nnnPerform comprehensive assessments, including history, physical exams, and risk evaluation for kidney disease.
nDiagnose and manage acute and chronic renal conditions, including but not limited to:nnChronic kidney disease (CKD)
nEnd-stage renal disease (ESRD)
nAcute kidney injury (AKI)
nGlomerulonephritis and nephrotic syndrome
nElectrolyte and acid-base disorders
n n
nOrder, interpret, and analyze diagnostic studies (labs, imaging, biopsies).
nDevelop and implement individualized treatment plans.
nInitiate and manage medications (e.g., antihypertensives, diuretics, immunosuppressants, SGLT2 inhibitors, etc.).
n nDialysis Management
nnnManage patients receiving hemodialysis and peritoneal dialysis.
nMonitor dialysis adequacy, tolerance, and complications.
nCollaborate with dialysis teams to adjust treatment prescriptions.
nAddress access-related issues (AV fistula, grafts, catheters) in coordination with nephrology and vascular surgery teams.
n nInpatient & Outpatient Rounding
nnRound on inpatient census in hospitals and rehab/SNF facilities for nephrology consults and follow-up.
nProvide consultations for renal-related conditions to hospital and rehab teams.
nManage transitions of care, including discharge planning and communication with outpatient providers.
nConduct outpatient clinic follow-ups and chronic disease management visits. Patient Education & Counseling
nEducate patients and families on:nnDisease progression and prevention
nMedication adherence
nDiet and fluid management
nDialysis options and transplant considerations

nPromote lifestyle modifications to slow CKD progression and improve overall health. Care Coordination
nCollaborate with nephrologists, primary care providers, hospitalists, dietitians, social workers, and dialysis staff.
nFacilitate referrals (transplant, vascular surgery, specialty care).
nEnsure continuity of care across inpatient, outpatient, and dialysis settings. Quality & Compliance
nParticipate in quality improvement initiatives, including Monthly Continuous Quality Initiative (CQI) meetings with group providers.
nEnsure adherence to regulatory standards and practice protocols.
nMaintain documentation in accordance with clinical, legal, and billing standards.
n nnQualifications
nnCurrent Texas Nurse Practitioner license (or eligibility to obtain).
nBoard certification as an NP or DNP (ANCC or AANP).
nDEA license and prescriptive authority in Texas (or eligibility).
nMinimum 1–2 years of clinical experience preferred but not required; dialysis, nephrology, or internal medicine experience strongly preferred.
n nnPreferred Experience
nn1–3 years nephrology or related experience (ICU, dialysis, internal medicine).
nFamiliarity with ESRD quality measures and CMS regulations.
nExperience with EMR systems.
n nSkills
nnStrong clinical judgment and advanced diagnostic interpretation.
nEffective communication with patients, families, and interdisciplinary teams.
nInterdisciplinary collaboration across hospital, clinic, and dialysis settings.
nStrong organizational skills and attention to detail.
n nnWork Environment
nnMixed practice across hospital, outpatient clinic, rehab/SNF, and dialysis unit settings with frequent travel between locations in the San Antonio area.
n nnSchedule and Coverage
nnFull-time position.
nTravel between multiple dialysis centers and practice locations in San Antonio and surrounding areas.
nRotating weekend call schedule with supervising physician assistance.
nRotational holiday coverage with supervising physician assistance.
nParticipation in monthly CQI meetings with group providers.
