Job description

Location: Location: Rochester, NY 14621

Schedule: Part-Time (2 days per week)

Total Annual Compensation: up to $70K

Job Summary: We are seeking a friendly and compassionate Nephrology Nurse Practitioner/ Physician Assistant to join our growing team. In this role, you will provide comprehensive nephrology care to patients in post-acute care settings while collaborating closely with physicians, nursing staff, and interdisciplinary care teams. This is an exciting opportunity to make a meaningful impact while working in a supportive and collaborative clinical environment focused on improving respiratory outcomes and enhancing patients quality of life.

Responsibilities: Conduct comprehensive assessment on patients with acute kidney injury and chronic kidney disease in post-acute care settings

Evaluate, diagnose, and manage acute kidney injury and chronic kidney disease

Develop and implement ind ividualized treatment plans

Collaborate with interdisciplinary teams

Educate patients, families, and facility staff on cardiovascular conditions, cardiac treatment plans, disease prevention, medication management, and continuity of care.

Monitor patient progress and adjust care plans as needed

Maintain accurate and timely EMR documentation

Support initiatives focused on reducing hospitalizations and Return-to-Hospital (RTH) events

Requirements Preferred a minimum of 6 months of experience in post-acute care setting

Preferred experience in Cardiology: training provided

Active and unrestricted state Nurse Practitioner license or Active PA license and NCCPA certification required

Master s Degree in Nursing from an accredited Nurse Practitioner program or Master s Degree from an accredited Physician Assistant program required.

CPR BCLS/BLS certified

Strong communication, organizational, and teamwork skills

Ability to work independently in the clinical environment
