Job description
UofL Health in Louisville, KY is seeking a Rehabilitation Nurse Navigator to coordinate care across the rehab continuum, from acute care through outpatient rehabilitation and community reintegration. The RN will educate patients and families, document care plans, and partner with physicians, therapists, and nursing staff to optimize outcomes.
Required ADN, 2 years rehab nursing experience, KY RN license, BLS, and PALS for pediatric care; BSN preferred.
