Responsibilities
Deliver patient-centered neurological care and support to Primary Care patients in the Home/Community setting.
Collaborate with the care team to deliver comprehensive and nurturing care.
Build community relationships and contribute to improving patient outcomes.
Qualifications
Active MD license in the state of SC
Excellent communication and interpersonal skills with the ability to build strong relationships at all levels of the organization
Strong problem-solving abilities
Ability to work effectively under pressure
