Job description
77 consultants seeks a qualified provider for exclusive coverage in Indiana. Coverage window: December 14, 2026, 7 AM to December 21, 2026, 7 AM, with potential extensions for March 8–15, 2027; May 31–June 7, 2027; and July–December 2027 (dates tentative).
Active Indiana license with board certification or eligibility required; minimum 2 years in the specialty preferred. Vaccination requirements (flu and COVID-19) with exemptions where allowed.
