Neurology Nurse Practitioner/Physician Assistant

Location: Hamden and Danbury

Schedule: Part-Time

Salary per annum: $30,000 - $60,000

Job Summary

We are seeking a friendly and compassionate Neurology Nurse Practitioner/Physician Assistant to join our team. As a Neurology Nurse Practitioner/Physician Assistant, you will play a crucial role in providing comprehensive care to patients with neurological disorders, including epilepsy, multiple sclerosis, Parkinson’s disease, and other neurological conditions. This is an exciting opportunity to make a difference in the lives of patients while working alongside a dedicated team of healthcare professionals.

Responsibilities

Develop and implement individualized treatment plans for patients, including medication management and non-pharmacologic interventions

Conduct regular follow-up visits to monitor patient progress, adjust treatment plans as necessary, and provide education and support

Collaborate with physicians and other healthcare professionals to coordinate care and ensure a multidisciplinary approach

Provide patient and family education on disease management, medications, and lifestyle modifications

Maintain accurate and timely documentation of patient encounters and treatment plans

Participate in quality improvement initiatives to enhance patient outcomes and optimize patient care delivery

Perform comprehensive assessments to accurately diagnose and treat patients with neurological conditions

Requirements

Minimum 6 months of experience in long‑term care or a skilled nursing facility

Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license

DEA License

CPR BCLS/BLS certified

Must hold a Master’s Degree in Nursing

Completed a degree from an accredited Nurse Practitioner program

A year of experience in Neurology

Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.): training provided

Must have great organizational and time management skills.

Must be able to lift a minimum of 20 pounds and stand / walk / work on your feet up to 6-8 hours per day
