Neurology NP Locums Job in Arizona Monday-Friday Days Beginning November 16 - Ongoing Assignment #
Coverage Details
Two providers needed
Credentialing timeframe: 60-90 days after receiving completed application with clean file
Open Dates
November 16-20
November 23-25, 27
November 30-December 4
December 7-11
December 14-18
December 21-24
December 28-31
January 4-8
January 11-15
January 18-22
January 25-29
Ongoing dates will begin Feb when schedules are released
Shift Details
Mon-Thursday: 8:00 AM - 4:30 PM
Friday: 8:00 AM - 12:00 PM with admin time 12:00 PM - 4:00 PM
Monday-Friday schedule
Lunch break 12:00 PM - 1:00 PM (paid time)
Requirements
NCCPA Certified
Arizona License or compact license with LOQ in hand at time of submission
Active ACLS, BLS - Highly Preferred - must be willing to obtain during credentialing
Clean malpractice/background
Local candidate - Preferred

Botox for migraines (if credentialed)
EMGs
Read EEGs
Assignment Details
Setting: Clinic
Support Staff: MA
Reason for coverage: Overflow
Patient population: Adult
Admissions: 20
Average clinic volume: Approximately 25 patients per day
