Job description
Atrinity Home Health, part of LHC Group within the Optum family, seeks a Physical Therapist to perform home health assessments, develop plans of care, and implement interventions to improve patient mobility and well‑being.
You will evaluate progress, document outcomes, and coordinate with physicians and the health care team to adjust treatment plans. A valid PT license, CPR, and transportation are required.
