Job description
Gentiva Hospice is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will assess patients, coordinate plans of care, and maintain clinical oversight during nights, weekends, and on-call periods.
You will work with the Interdisciplinary Group to ensure seamless communication with physicians, families, and care teams, and contribute to documentation, audits, and compliance activities. A $7,500 sign-on bonus is offered.
