Job description
Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard business hours, delivering assessments, admissions, routine visits, and live discharges with primary clinical oversight.
You will work nights, weekends, and on-call hours, participate in the IDG, educate families, maintain documentation, and support compliance and survey readiness while upholding compassion and teamwork.
