Job description
Heartland Hospice, part of Gentiva, seeks an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. The role offers primary clinical oversight, on-call coverage, and coordination with the IDG to ensure seamless patient care and smooth family support across nights and weekends.
You will perform assessments, manage plans of care, educate families, and maintain compliant documentation while leading field staff and participating in ongoing trainings.
