Job description
Gentiva Hospice is seeking an Afterhours RN Care Manager to provide compassionate hospice care during nights, weekends, and on-call hours. You will assess patient and family needs, maintain clinical oversight, and collaborate with physicians and the IDG to ensure seamless care delivery.
The role requires an active RN license in the state, CPR certification, and at least one year of RN experience in hospice or related settings. Strong communication and leadership skills are essential.
