Job description
Gentiva is seeking an Afterhours RN Care Manager to deliver high-quality hospice care outside standard hours. The role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call shifts.
You will assess needs, develop care plans, educate families on disease progression and symptom management, and maintain compliance and documentation while collaborating with physicians and the IDG.
