Job description
Gentiva Heartland Hospice seeks an Afterhours RN Care Manager to provide high‑quality hospice care after hours, weekends, and on‑call periods. You will oversee clinical care, assess patients, and coordinate with physicians and the IDG to ensure continuity and quality.
You will lead field staff, document thoroughly, and ensure compliance with hospice policies while educating families about care and symptom management.
