Job description
Gentiva Hospice is seeking an Afterhours RN Care Manager to oversee in-home hospice care during nights, weekends and on-call hours. This role provides direct patient care, conducts assessments, and maintains primary clinical oversight for assigned patients.
You will collaborate with the IDG, update care plans, educate families, ensure documentation, and support regulatory compliance. A current RN license and at least one year of hospice/home health or acute care experience are required.
