Job description
Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive in-home health assessments for members across the state. Visits can last up to an hour and involve review of medical history, medications, social factors, and care needs.
Providers will conduct assessments at members' residences or SNFs, order tests as needed, and coordinate with PCPs and care teams. Strong clinical skills, travel readiness, and excellent communication are essential.
