Job description

Bluestone Physician Services is seeking a Nurse Practitioner or Physician Assistant to provide on-site primary care in Assisted Living and Memory Care communities around Mankato, MN. You will join a multidisciplinary team delivering proactive, patient-centered care with an emphasis on quality outcomes.

The role includes scheduling flexibility, a patient panel of approximately 12–16 daily visits, and collaboration with LPNs, RNs, and social workers to manage chronic conditions and transitions
