Job description

UnitedHealth Group in Yonkers, NY is hiring Senior Advanced Practice Clinician within the SCC team to provide care to high‑risk members in long‑term care settings, with no on‑call, weekends, or holidays required. Responsibilities include comprehensive assessments, care planning, documentation, and collaboration with the interdisciplinary team to support optimal outcomes.
