Job description
Great opportunity in Salem, OR for an NP/PA to perform Primary Care in a clinic and Annual Wellness Visits in-home working for a private group.
Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP) or Physician Assistant (PA)
2+ yrs exp as NP required - Primary Care, Family Med, Internal Med, Gerontology, or Palliative Care exp. Home Care exp is a plus
Duties will include working in a clinic and performing annual wellness exams for senior Medicare patients in their home
This role is a mix of outpatient in-clinic setting, in-home health visits, and occasional telehealth appointments
Collaborate with Primary Care team to ensure completion of holistic patient care
Must have active OR medical and drivers license, active DEA, currently enrolled in Medicare and in good standing, have HCC coding exp, and have Annual Wellness Visits experience
40 hours a week - Mon-Fri 8a-5p
