Bluestone Physician Services is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to provide on-site, team-based primary care in Assisted Living and Memory Care communities around Maple Grove, MN. Full-time schedule Mon-Thu with an optional Friday, managing a patient panel and collaborating with LPNs, RNs, and Social Workers.
