Job description
UnitedHealth Group is seeking an Advanced Practice Clinician (NP/PA) to join the Senior Community Care (SCC) team. You will conduct comprehensive assessments, coordinate care for high-risk members, and collaborate with an interdisciplinary team to improve health outcomes across nursing homes, senior housing, and home care settings.
Qualifications include ANCC/AANP or NCCPA certifications, active state licensure, DEA authority, and a valid driver's license.
