Job description
Salud Family Health in Fort Collins, CO is seeking a full-time Nurse Practitioner or Physician Assistant to deliver care in a team-based medical home. The role involves collaborating with medical, dental, behavioral health, and clinical pharmacy teams to coordinate patient care.
The ideal candidate will have graduated from an accredited NP/PA program and hold an unrestricted Colorado license; Spanish language ability is strongly preferred.
