Job description
MetroPlus Health is seeking a Nurse Care Manager to lead care management for medically fragile children, reviewing plans of care and UM requirements. The role involves collaboration with Health Home Coordinators, LDSS/VCFA, and clinicians to ensure appropriate access and level of care.
The position requires a NYS Registered Nurse license, a minimum bachelor’s degree, and at least two years of acute clinical experience, with knowledge of HCBS services and InterQual Guidelines.
