Job description
Your Health Organization in the Columbia area seeks a Registered Nurse to serve as a Nurse Case Manager. You will visit patients in facilities, homes, and ALF/ILF sites, coordinating with providers and initiating telemedicine visits. Daily travel is required with a 7 on/7 off, 12-hour shift track.
