nnnStarting Salary: $80,000 annually

nnSchedule: Monday–Friday, 8:30 a.m.–4:30 p.m.
n
nnLeadership: Directly supervises the LPNs assigned to the homes
n
nnOn-Call: Rotating on-call responsibilities required

nSUMMARY
nThe RN Case Manager’s primary role is developing and implementing care plans for their clients that coordinate the clients' home care services and that follow physicians' orders. In addition, the Case Manager ensures that services are delivered according to licensing guidelines, professional medical standards and agency policies and procedures.
nESSENTIAL JOB FUNCTIONS
nThe requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job:
nnnPerforms client intake assessments including submission of required documentation for prior authorization of client services as needed based on client's pay sources.
n
nnDevelops a plan of care based on nursing diagnosis, client and physician input that includes medical interventions and measurable goals or outcomes.
n
nnRoutinely reviews pertinent client medical data to determine the effectiveness of services in reaching maximum rehabilitation potential.
n
nnInvestigates and takes appropriate actions on client or employee concerns/complaints.
n
nnEnsures that aides are properly oriented and trained to meet the client's needs in accordance with agency policies and procedures.
n
nnPerforms periodic supervisory visits for aides with a written evaluation regarding service delivery.
n
nnInstructs clients, family members and primary care givers as needed to ensure the maintenance of or acquisition of optimal functioning level for each client.
n
nnEnsures effective coordination of client home care services through the timely completion of required documentation as well as the timely transfer of pertinent medical data to the client's physician, therapists or other care givers.
n
nnProvides input for formulation or modification of agency policies, procedures, and practices pertaining to client services.
n
nnParticipates on the advisory board or other agency quality assurance committees as requested.
n
nnPerforms other duties and actions as as required.

n nMinimum Knowledge and Skills required by the Job

nEducation and Experience:
nnnBachelor’s Degree in Nursing

nnAt least two years’ experience.

n nCertificates, Licenses, and Registrations:
nnValid nursing license in state of work.

nnTravel as needed
n nPhysical Requirements
nnHeavy work. Exerting up to 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects.
