Job description
Decypher seeks a Registered Nurse – Case Manager to coordinate patient care across the continuum, serving as an advocate and case manager for complex, high-risk cases. You will work with a multidisciplinary team to support service members, veterans, and families, aligning referrals, utilization management, and resource access.
