Job Title: Nurse Case Manager

nJob Description

nThe Nurse Case Manager supports care coordination, administrative nursing functions, and operational workflows in a provider or payer environment. This non-clinical, non-patient-facing role focuses on facilitating communication between care teams, supporting population health programs, and contributing to process and quality improvement initiatives that align with organizational goals. Working within a team-based setting, the Nurse Case Manager enhances internal care processes through coordination, documentation, and collaboration, without performing direct patient care, invasive procedures, or exercising clinical judgment.

nResponsibilities

nnnCoordinate care management activities through administrative support and structured workflows.

n

nnCollaborate with providers, care teams, and external partners to support continuity of care processes.

n

nnReview and manage documentation, referrals, and care plans based on established guidelines.

n

nnProvide general health education and resource information using approved materials.

n

nnRoute patient and provider inquiries to appropriate clinical or operational teams.

n

nnSupport workflow efficiency, scheduling alignment, and internal operational processes.

n

nnServe as a resource to internal teams on systems, coordination workflows, and administrative procedures.

n

nnContribute to quality improvement initiatives and assist with related reporting.

n

nnAssist with onboarding coordination, scheduling, and team support functions.

n

nnParticipate in internal meetings and organizational initiatives to enhance care coordination and operations.

n

nnPerform additional administrative duties as assigned to support healthcare operations.

n

nnMaintain compliance with all organizational policies, procedures, and regulatory requirements.

n

nnMaintain regular attendance and punctuality in accordance with organizational standards.

n

n nEssential Skills

nnnActive, unencumbered Registered Nurse (RN) license, with Kansas licensure preferred.

n

nnGraduate of an accredited RN program.

n

nnStrong communication skills to effectively interact with providers, care teams, internal staff, and external partners.

n

nnProven coordination and organizational abilities to manage documentation, referrals, and care plans.

n

nnAbility to work in a structured, fast-paced environment with clear workflows and performance expectations.

n

nnStrong time management and problem-solving skills to prioritize tasks and resolve operational issues.

n

nnCultural awareness and the ability to support diverse populations in a respectful and inclusive manner.

n

nnUnderstanding of payer and provider operations or population health concepts (preferred).

n

nn2+ years of experience in care coordination, case management, utilization review, or healthcare operations preferred.

n

nnExperience in payer, outpatient, or administrative healthcare environments preferred.

n

nnComfort with heavy use of computers, phones, and healthcare systems such as EMR/EHR platforms.

n

n nAdditional Skills & Qualifications

nnnExperience providing care coordination and utilization review support.

n

nnPrior leadership or team support experience is a plus.

n

nnAbility to serve as a resource on systems, workflows, and coordination processes.

n

n nWork Environment

nThis is an on-site, office-based role within a provider or payer organization, scheduled Monday through Friday, 8:00 AM to 5:00 PM. The environment is fast-paced and team-oriented, with a strong focus on coordination and administrative support rather than direct patient care. The role involves heavy use of computers, phones, and healthcare information systems such as EMR/EHR platforms, and regular collaboration with internal teams, providers, and external partners. There is no direct patient care, no clinical procedures, and no exposure to clinical treatment environments. Work follows structured workflows and clearly defined performance expectations aligned with operational goals, within a collaborative culture that emphasizes mission-driven work, community impact, and a competitive benefits package that supports professional development and overall well-being

nThis is a Contract position based out of Olathe, KS.

nPay and Benefits

nThe pay range for this position is $25.00 - $35.00/hr.

nIndividual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
