Emergency Department Case Manager

This position is located in the Kayenta Service Unit, Nursing Services, Emergency Department, Kayenta, AZ. The primary purpose is to provide Emergency Department case management and clinical coordination through patient assessment, care coordination, service integration, follow-up, and evaluation. The position supports continuity of care, improves patient flow, and ensures safe, efficient, and effective healthcare delivery. A REAL ID will be required in accordance with 6 C.F.R. 37.5 (2021).
