Job description
Harris Health System, Inc. seeks a Nurse Clinician for Hospital at Home to deliver high-quality, in-home acute care. The role requires independent management of common patient problems and coordination of HaH enrollment across the system.
The NC-HAH will organize in-home visits, utilize scheduling tools, and apply evidence-based standards to care delivery while upholding professionalism and Service First behaviors.
