Job description
Harris Health System, Inc. is seeking a Nurse Clinician, Home Division (NC-HD) with the skills to deliver high‑quality acute care in patients' homes and in Harris Health facilities.
The role covers Hospital at Home, House Call Service, OPAT, and additional Home Division services to meet patient care needs. The NC-HD coordinates care, educates patients, and prioritizes in-home acute and ambulatory visits, using evidence‑based resources and professional standards while maintaining professionalism
