Job description
Harris Health System, Inc. seeks a Nurse Clinician, Hospital at Home (NC- HaH) to provide high-quality in-home acute care across the Harris County region.
The role leverages evidence-based guidelines and a scheduling system to deliver timely, patient-centered visits while coordinating HaH enrollment for eligible patients. The incumbent will monitor clinical outcomes, participate in quality improvement projects, and uphold a customer-friendly Service First approach with patients and families.
