Duties
Help
Develop, implement, and monitor MTF's clinical case management.
Coordinate care and collaboration between multiple military and/or civilian (purchased care) healthcare agencies.
Evaluate clinical case management activities and conducts ongoing monitoring and evaluation of the Clinical Case Management Program using objective criteria.
Educate the patient and family on the clinical case management process.
Provide robust and comprehensive clinical case management, care coordination, and discharge/disposition services.
