About This Role
The Nurse Director, Care at Home Palliative Care is responsible for establishing best practice models and designing patient care delivery systems. This role is ideal for a leader who can partner with healthcare teams to achieve optimal patient care at home.

What You’ll Do
Establish best practice models and design patient care delivery systems.
Lead the investigation of patient/family/member concerns regarding care.
Partner with physicians and healthcare providers to ensure safe patient care.
Manage resource needs in clinical areas for appropriate assignment and utilization.
Direct the use and maintenance of equipment, supplies, and medications.
Assume accountability for patient and employee safety.
Maintain clinical expertise and provide supervision and competency assessments.
Delegate tasks aligned to scope of practice.
Oversee the operation of multiple units within a department.
Direct continuum of care operations to support patient care.
