nPosition Overview
nThis position is responsible for providing nursing services to children and youth, medical consultation to FCCP staff and foster families, and following the mandated and recommended health services activities needed to support optimal health for children in foster care. Nurses must comply with Early and Periodic Screening, Diagnostic and Treatment (EPSDT) standards and state regulations while performing functions noted below.

n nKey Responsibilities
nnnConduct comprehensive health assessments, including HIV and intake assessments, first aid, triage, substance abuse screenings, developmental and mental health evaluations, and physical examinations for signs of abuse.
n
nnProvide health education and training to children, caregivers, and staff, including reproductive health education and wellness promotion.
n
nnEducate and support caregivers by monitoring healthcare needs and participating in home visits to ensure continuity of care.
n
nnAdminister and manage medications in accordance with established policies and procedures.
n
nnCoordinate community healthcare services, facilitate medical appointments, and ensure appropriate follow-up for all health-related needs.
n
nnServe as a liaison between the voluntary agency and Local Department of Social Services (LDSS), ensuring timely communication regarding children's health concerns, safety, and well-being.
n
nnDevelop individualized treatment goals based on assessments, screenings, medical history, and collaboration with an interdisciplinary team.
n
nnMonitor and maintain accurate documentation within the CONNECTIONS Health Services Module, ensuring records are timely, comprehensive, organized, and compliant with regulatory standards.
n
nnMaintain health records and files to support continuity of care, oversight, and quality assurance requirements.
n
nnImplement infection control protocols and ensure adequate nursing coverage, medical supplies, and equipment availability.
n
nnProvide 24/7 on-call support for urgent health concerns involving children and youth and respond to inquiries from State, City, and County authorities as needed.
n
nnCollaborate with multidisciplinary teams to address the physical, developmental, behavioral, and emotional health needs of children and families.
n
nnPerform additional duties and special projects as assigned by management.

n nQualifications
nnnSatisfactory completion of Mandated Reporter Training.
n
nnLicensed Practical Nurse or Registered Nurse required.
n
nnFirst Aid and CPR certification required.
n
nnObtain satisfactory background checks pursuant to NY Health Care Facility requirements.
n
nnMust be at least 21 years of age.
n
nnValid State Driver's License required.
