Position Summary

The Licensed Practical Nurse (LPN) is responsible for supporting the health, safety, and overall well-being of residents through the delivery of high-quality nursing care. This role combines clinical expertise with compassionate support, ensuring residents receive the medical attention, education, and encouragement needed to achieve their recovery goals.

Essential Duties & Responsibilities

Administer medications, treatments, blood glucose monitoring, and other physician-prescribed care in accordance with state regulations and facility policies.

Monitor resident health status and promptly identify changes in condition.

Measure and record vital signs, including blood pressure, pulse, temperature, and weight.

Maintain accurate and timely documentation in residents' medical records.

Provide emotional support and therapeutic communication to residents.

Educate residents regarding medications, treatments, and health-related concerns.

Communicate abnormal findings, incidents, behavioral changes, lab results, and medication reactions to physicians and other members of the healthcare team.

Respond effectively to medical emergencies and provide necessary nursing interventions.

Participate in the development and implementation of resident care plans.

Collaborate with interdisciplinary team members to ensure quality resident outcomes.

Qualifications

Required

Current Licensed Practical Nurse (LPN) license in good standing in the State of Illinois.

Current CPR Certification .

Strong communication, documentation, and interpersonal skills.

Commitment to providing compassionate, resident-centered care.

Preferred

2–3 years of nursing experience.

Previous experience working in behavioral health, psychiatric, long-term care, or residential settings.

Experience with care planning and interdisciplinary team collaboration.

Schedule Opportunities

We are currently hiring for the following 12-hour shifts:

Day Shift: 10:00 AM – 10:30 PM

Night Shift: 10:00 PM – 10:30 AM

Schedule includes every other weekend.
